When we think about illnesses, these include conditions which may cause damage to any part of the body, whereas ill health conditions concern those that might lead to changes in the mind. Another way of thinking about illnesses is to separate those that are acute, where the symptoms occur suddenly but are temporary, and those that are , which are long-lasting and progressive conditions.
Individuals who are experiencing ill health might receive care and treatment in such as hospitals, GP surgeries, the workplace, care homes and in their own homes. In complex cases, these service providers will work together to meet the needs of the patient in teams. As an example, one care professional will carry out an assessment of the patient’s needs and will then pass on certain information to relevant care workers in the other organisations.
